Personal Assistance Services (PAS)

Personal Assistance Services are non-medical, hands-on support services that help individuals with daily living activities they might otherwise struggle to do on their own due to age, disability, or illness.

Our caregivers are matched to each client's personality, language preference, and care needs. Services are delivered according to a written, individualized plan of care — reviewed and updated as needs change.

What PAS May Include

  • Bathing, dressing, and grooming assistance
  • Mobility support and transfer assistance
  • Meal preparation and dietary support
  • Light housekeeping related to client care
  • Companionship and safety supervision
  • Errand assistance and accompaniment

Broad Coverage

Home & Community Support Services

As a licensed Home and Community Support Services Agency (HCSSA), Lone Star Health Services is authorized to provide a broad spectrum of home-based and community-based care services under the Texas regulatory framework.

Services are delivered in accordance with each client's individualized plan of care — developed in coordination with the client, their family, and where applicable, a physician or case manager.

Service Delivery Includes

  • Care delivered in the client's residence
  • Community-based support per the plan of care
  • Coordination with physicians and case managers
  • Regular supervision and quality checks
  • Documentation and compliance per HHSC standards

We do not provide clinical nursing, therapy, or skilled medical services. Specific scope is always confirmed per client and payer requirements.

How It Works

From Referral to First Visit

Referral or Direct Contact

Families, physicians, hospital discharge planners, or case managers contact us to inquire about services.

Assessment

We conduct a needs assessment to understand the client's functional needs and document the individualized plan of care.

Caregiver Match

We match the client with a screened, trained caregiver — taking into account language, personality, and availability.

Services Begin

Care starts per the authorized schedule. We check in regularly to ensure quality and adjust care as needed.

Important Notes

Scope of Services

Lone Star Health Services does not provide skilled nursing, physical therapy, occupational therapy, or other medical/clinical services. We provide non-medical personal assistance and community support. Clients needing clinical care should ask their physician for a home health agency referral.

Authorization Required

Services delivered under Medicaid managed care programs require prior authorization from the client's managed care organization (MCO). We work with families and case managers to facilitate this process. Please call us to understand current authorization requirements.

Individualized Plans of Care

Every client receives a written plan of care outlining the specific services to be provided, the frequency of visits, and any special instructions. This plan is developed collaboratively and updated as the client's needs evolve.
