How We Start Care: 6 Steps

Here's exactly what happens from your first contact to the first caregiver visit.

  • Step 1 — Initial Contact

    Call us at (201) 456-3103, email us, or submit the care request form below. A care coordinator will respond within one business day to discuss your situation, answer questions, and determine if our services are a good match.

  • Step 2 — Insurance & Authorization Verification

    We collect the client's insurance information and work to verify coverage. If services are to be billed to a managed care organization (MCO), we assist in understanding the prior authorization process. Private pay options are also discussed at this stage.

  • Step 3 — In-Home Assessment

    A representative from our agency visits the client's home to conduct a needs assessment. We evaluate daily living activities, safety considerations, and the client's preferences — including language and cultural needs. This assessment forms the basis of the plan of care.

  • Step 4 — Plan of Care Development

    Based on the assessment, we develop a written individualized plan of care. This document outlines the specific services to be provided, the schedule, and any special instructions. The client or their authorized representative reviews and signs the plan.

  • Step 5 — Caregiver Matching & Assignment

    We match the client with a caregiver who meets their language preference, personality, and care needs. All caregivers undergo background checks and training before assignment. Families are introduced to the caregiver before the first visit.

  • Step 6 — Ongoing Care & Supervision

    Once care begins, our supervisory staff conducts regular check-ins — both with the client/family and through supervisory visits — to ensure quality and address any concerns. The plan of care is updated as needs change.

All caregivers undergo a background check prior to placement. Our screening process is designed to meet Texas HHSC regulatory requirements for home care workers. Additional screening steps are taken consistent with payer requirements and best practices in the industry.
We serve Harris County (our primary service area) plus the surrounding SDA counties: Austin, Brazoria, Fort Bend, Galveston, Matagorda, Montgomery, Waller, and Wharton. If you're unsure whether your area is covered, call us and we'll confirm.
Start times vary depending on authorization requirements and caregiver availability. For private-pay clients, we aim to begin the intake process immediately and start services as quickly as possible. For MCO/Medicaid-funded clients, care can begin once prior authorization is in place. Contact us to discuss your timeline and we'll do our best to accommodate urgent needs.
We accept private pay, and are working to establish contracts with managed care organizations and other payers.
